Rock Island Public Library Physical Library Card

Online application and requirements

By applying or renewing online for either a Full Access Library Card (ages 6 and up) or the My First Library Card for my child (ages birth through 5), I confirm that:
  • I believe that I live in your legal service area of either the City of Rock Island, or the Milan-Blackhawk Area Public Library District
  • I realize this card is for my use only (or in the case of a child's card, for that child.)
  • I will report loss or theft of my card to the Library Circulation Department at 309-732-7323 or Email Circulation Department
  • I will give prompt notice if I change my address or other contact details. (If you leave our service area, you will need to apply for a new card with the library in your new city/town of residence.)
  • I understand that the right to use the library card issued as a result of this application obligates me to comply with all library policies as from time to time amended, and to pay for loss or, or injury to, materials and equipment borrowed, to pay fines for overdue materials and equipment, or any other reasonable fees. Confidentiality requirements are hereby waived to the extent necessary for collection of overdue fines, or loss, or injury to materials or equipment.
  • I realize that the library is not responsible for any damage or harm a borrowed item may cause to persons or personal property.
